When to ApplyProposals must be submitted via FAX or email by May 1st for consideration at the June meeting, or by October 1st for consideration at the November meeting. Proposals sent by mail will be considered if they are post-marked by those dates. We accept submissions by email, fax, or postal mail, and we will notify you when we receive your application. The Foundation does not consider more than one proposal from the same organization within a twelve-month period, unless that organization is applying for an Impact Grant.
How to Apply A complete proposal must contain one copy of each of the following: 1. Required Information Sheet: a very important form that gives us a “snapshot” of your organization, your project, and the people you benefit. Click HERE to take you to a page with the downloadable Required Information Sheet. We suggest you copy and paste this form into your own program, such as Word.doc . Then you can fill it out and re-attach to an email, or print it out and send it to us by one of the following methods.
2. Do you have a website? If so, be sure to include the URL to give us a more complete picture of your organization. 3. Executive Summary: a brief (up to 2 pages) letter giving us some background information about your organization, and summarizing your grant request. 4. Your organization’s 501(c)(3) form from the Internal Revenue Service. We cannot consider your organization without this document. 5. Board of Directors and Funding Sources: please list your organization's board of directors, as well as your other funding sources for this request. Please list whether this funding is committed, pending, or received. 6. Supporting Documentation: 2-3 items of current printed materials, such as annual reports, media clippings, testimonial letters, brochures, etc. Additional Questions?
